Family ID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs and helps us be administratively efficient and environmentally responsible. When you register with Family ID all information is saved in your personal Family ID profile. You only need to enter information once for each family member. Family ID takes your entry and auto populates for all forms and multiple programs across our system. Clyde-Savannah Family ID Registration Page At any time, you may login at Family ID to update your information and to check your registration(s). If you need assistance with registration, contact Family ID at: Support Family ID or 888-800-5583 x1. Support is available 7 days per week and messages will be returned promptly. New York State mandates that an annual physical completed within 12 months prior to the start of practice be on file at the student athlete’s school. If your child has not had a physical in the past 12 months, please contact their PCP (Primary Care Physician) as soon as possible to schedule an appointment. In the event, that your child is without insurance or other factor that delays a physical being completed prior to the first day of practice, our School Medical Director, may be able to do the physical at Clyde-Savannah High School. Student-athletes must be signed up on Family ID prior to their physical being completed at the school. Sports Physicals: Please contact Andrea Roelle (HS nurse's office for more information) Ideally, your PCP should be the provider who sees your child for their annual physical, there may be important items they need to discuss such as medications, lab work and/or immunizations our School Medical Director is unable to do during this time.  Any student who has an underlying health condition or taking routine or rescue medications MUST see their PCP for the annual exam. If your child was not able to participate in PE or Clyde-Savannah Interscholastic Athletics at the end of the school year; you will need documentation from your respective doctor showing that they are cleared to play a Fall Sport. Also, please bring any new orders you may have such as for an inhaler, EpiPen etc.  Any questions/concerns, please contact the building nurse or our athletic department: Clyde-Savannah High School
